Session Description
This session builds on “Caring for the Most Vulnerable” by highlighting how a multidisciplinary team works together to improve health outcomes for children in foster care. The Children’s Foster Care Program integrates best practice for medical, behavioral and social care using a trauma-informed, child-centered approach. Attendees will explore how this team collaborates with system partners to navigate the complex needs of this population to provide comprehensive, accessible and consistent health care. This session aims to provide insight for professionals across agencies working to deliver more effective, coordinated care for foster youth.
